HOSPICE CARE

Routine Care

We create a customized plan of care aimed at meeting your daily needs. The plan of care reflects a comprehensive support approach, which fulfills your physiological, spiritual and sociological needs. In addition, we evaluate all your material needs as well.

Medication and Equipment are ordered ahead of time to ensure a smooth transition in the Program.

Our interdisciplinary team will discuss your needs with you and your Caregivers. We will also be 24/7 to make sure you are comfortable and pain free. Our team is aware of anxiety and/ or panic moments that the patient and the family may experience.
For that reason, our Hospice team will be available to you seven days a week, twenty-four hours a day.

Inpatient Care

Inpatient Care can be offered for the following reasons:

Symptom Management or General Inpatient Services: When your loved one experiences pain and/or other symptoms that can’t be controlled at home, the patient is transferred to a skilled facility where s/he will receive 24/7 hour care. The facility will provide the medication and equipment needed for the patient. The Hospice team will visit the patient daily to make sure that all is going as it should, and that the symptoms of the patients are controlled.

Respite Care: Caring for a Hospice patient can take a toll on caregivers. Thus, Medicare and most insurance companies allow for the family to take a break on a quarterly basis.

During that break, the patient is transferred to a facility while still receiving Routine Hospice Care. Our Hospice team will still provide the scheduled routine visits and ensure that the patient is comfortable, and psychologically stable.

Our Hospice team will be available to you seven days a week, twenty-four hours a day.

Continuous Care

Sometimes, a patient may elect to receive continuous care at home. During Continuous care, the patient receives hospice care consisting predominantly of nursing care on a continuous basis at home. Home health aide or homemaker services or both may also be provided on a continuous basis. Continuous home care is only furnished during brief periods of crisis and only as necessary to maintain the terminally ill patient at home.

Bereavement Support

13 months following the passing of a loved one, Serenity Home Hospice will provide support through its Bereavement program. Serenity Home Hospice will also help the family remember their love one, via memoriam ceremonies.
