Our role at Serenity is to be the emotional, psychological,andclinical companion to all our patients and their caregivers.

During these challenging times, we seek to the beauty and quality of life that is still present until gone.

Our clinicians are highly trained professionals. They include Physicians, Registered Nurses, Certified Nurses Aides, Master Social Workers, Chaplains and Bereavement counselors who are dedicated to the care of your loved one.

At Serenity, we are aware of the uneasy times the family of the patient is going through. That is why our team designs a comprehensive plan of care that is inclusive
of the whole family.

If your loved one is in need of symptom Management, our Hospice team will seek for placement in a Skilled Nursing facility, where the symptom can be controlled. Our
Hospice Nurse will visit the patient daily to ensure that the proper care protocol is being administered to the patient.

Whether the care is provided at home, or in a facility, Serenity will always make sure that Comfort,Quality of Life and Peace of Mind are attained.

Eligibility Requirements

To qualify for hospice care, the patient must have a doctor’s signed diagnosis of a terminal illness with six months or less life expectancy, a decision to forgo any curative treatment, a willing and able caregiver (family member or friend) when unable to be alone, and a preference to die at home.

A nurse will visit with you and your family to answer your questions about hospice care before a decision must be made to choose this type of care. It is recommended that anyone you want involved in making this decision be present during this informational visit so all questions can be answered to your satisfaction.
